The participants will need a water bottle, football boots (no full studs or flat shoes) to wear at the SWFC Community Facility (Clay Wheels Lane, S6 1LZ) and appropriate footwear for the remainder of the day.
Please note, the half-time mini-game is subject to weather conditions. We may, unfortunately, not be able to play on the pitch on the day.
The match fixture is subject to change. In the event of the game being moved to an alternative day, we will contact you immediately. Your booking will be moved to the new date and time. If the new date isn’t suitable, we will of course issue you with a refund.
We require the balance to be paid 1 month prior to the fixture.
For parents/guardians of the participants wanting to purchase tickets or to change their season ticket to sit with the party please go into the ticket office the week running up to the party. It will not be possible to purchase the tickets before that week. Quote the birthday name when doing this.
Please do not go in on the matchday to purchase or change tickets as it gets extremely busy which could cause you delays getting into the stadium.

Matchday Party Consent Form
This consent form should be completed by the parent or person who has legal guardianship of any child, under 18 years, who will be involved in activities associated with Sheffield Wednesday Football Club.
By consenting to this I am stating that my child is in good health and that he/she is not participating contrary to medical advice.
In the unlikely event of an accident occurring, I give my permission for a designated representative to authorise emergency medical treatment, including the use of anaesthetic if deemed necessary.
As the legal guardian of the child, I agree that the images taken may be used in publicity materials and celebration publications produced by the EFL, League Football Interactive, the EFL Trust, League Football Education and/or the EFL’s sponsor’s Sky Bet or other/future EFL commercial partners/sponsors. This will include images in press releases, on websites or social network sites managed by any of those groups.
I understand that in addition, the images of my child may appear on live or recorded televised footage and understand any images will be safely stored and archived digitally or manually, in line with The Data Protection Act and that these may be used at any time for promotional or publicity purposes.
